ABSTRACT

Objetivo: Describir los resultados de la blefaroplastia de los párpados superiores realizada con radiocirugía a pacientes con dermatochalasis del párpado superior. Métodos: Se realizó un estudio descriptivo longitudinal con pacientes del servicio de Oftalmología en el Hospital Clínico Quirúrgico Hermanos Ameijeiras en el período entre enero de 2021 a diciembre de 2022. Después de aplicar los criterios de selección la muestra quedó constituida por los primeros 50 pacientes que asistieron de forma consecutiva. Resultados: Predominaron los pacientes entre los 60 a 79 años de edad, el sexo femenino y los de color de piel blanca (p > 0,05). Se logró una reducción completa de la piel redundante en más de un 50 %. El tiempo quirúrgico promedio con radiofrecuencia fue de 35.9 min. La satisfacción de los pacientes fue buena en un 96 %. Las complicaciones posoperatorias se manifestaron en un 32 % de los pacientes, solo en dos pacientes fueron serias, las cuales no dejaron secuelas. Conclusiones: La cirugía con ondas de radios para el tratamiento de la dermatochalasis permite alcanzar el resultado estético que se desea, con satisfacción tanto para el médico, como para los pacientes quienes logran mejorar su calidad de vida.


Objective: To describe the results of upper eyelid blepharoplasty performed with radiosurgery in patients with upper eyelid dermatochalasis. Methods: A longitudinal descriptive study was carried out with patients from the ophthalmology service at Hospital Clínico Quirúrgico Hermanos Ameijeiras, from January 2021 to December 2022. After applying the selection criteria, the sample was made up of the first 50 patients who attended consecutively. Results: Patients aged 60-79 years predominated, as well as the female sex and the white skin color (p > 0.05). A complete reduction of redundant skin was achieved in more than 50 %. The average surgical time with radiofrequency was 35.9 min. Patient satisfaction was good in 96 %. Postoperative complications were present in 32 % of patients, only in two patients were they serious, but did not leave any sequelae. Conclusions: Radiowave surgery for treating dermatochalasis allows to achieve the desired aesthetic outcome, with satisfaction for both the physician and the patients, who achieve an improvement in their quality of life.

Objetivo: Describir los resultados de las blefaroplastias con láser de CO2 y radiocirugía realizada a pacientes con dermatochalasis del párpado superior en el Hospital "Hermanos Ameijeiras". Métodos: Se realizó un estudio descriptivo longitudinal en pacientes del servicio de Oftalmología. Después de aplicar los criterios de selección la muestra quedó conformada por los primeros 100 pacientes que asistieron de forma consecutiva y se dividieron al azar en dos grupos de 50 casos cada uno, al grupo A se le realizó blefaroplastia superior transcutánea con radiofrecuencia y al grupo B se le realizó blefaroplastia superior transcutánea con láser de CO2. Resultados: En ambos predominaron los pacientes entre los 60 a 79 años, el sexo femenino y los de color de piel blanca (p>0,05); se logró una reducción completa de la piel redundante en más de un 50 pr ciento. El tiempo quirúrgico promedio con el láser de CO2 fue de 31.5 min y con radiofrecuencia de 35,9 min. El sangrado intraoperatorio fue menor en operados con el láser de CO2. La satisfacción de los pacientes fue buena en ambos grupos. Complicaciones posoperatorias: el 16 del grupo A y el 11% del B, presentaron alguna complicación. Conclusiones: Se observó que con el uso del láser de CO2 el tiempo quirúrgico, el sangrado intraoperatorio y las complicaciones disminuyeron; con lo cual mejoró el aprovechamiento del salón. Ambas técnicas mostraron un elevado grado de satisfacción y mejoría de la estética palpebral por lo que son efectivas y seguras(AU)


Objective: To describe the results of CO2 laser blepharoplasty and radiosurgery performed in patients with upper eyelid dermatochalasis. Methods: A descriptive longitudinal study was carried out in patients of the Ophthalmology Service of the Clinical Surgical Hospital "Hermanos Ameijeiras". After applying the selection criteria, the sample consisted of the first 100 patients who attended consecutively and were randomly divided into two groups of 50 cases each. Group A underwent transcutaneous upper blepharoplasty with radiofrequency and group B underwent transcutaneous upper blepharoplasty with CO2 laser. Results: In both groups, patients between 60 and 79 years of age, female gender and white skin color predominated (p>0.05); a complete reduction of redundant skin was achieved in more than 50 percnt. The average surgical time with CO2 laser was 31.5 min and with radiofrequency 35.9 min. Intraoperative bleeding was lower in those operated with the CO2 laser. Patient satisfaction was good in both groups. Postoperative complications: 16 por ciento of group A and 11 percent of group B presented some complication. Conclusions: It was observed that with the use of CO2 laser the surgical time, intraoperative bleeding and complications decreased; thus improving room utilization. Both techniques showed a high degree of satisfaction and improvement of palpebral aesthetics, so they are effective and safe(AU)

ABSTRACT

Los ojos son el foco principal de la cara y se les atribuye la expresión de la personalidad del individuo. La configuración anatómica de los párpados y de la región periorbitaria conforma una de las áreas más importantes de la expresión humana. Existen alteraciones aparentemente estéticas que llevan a trastornos visuales, como la dermatocalasis y blefarochalasis, que limitan el campo visual o provocar fatiga ocular por la presión de la piel redundante sobre los párpados, la laxitud de los tendones cantales origina un síndrome de exposición escleral y a la vez lagrimeo. La blefaroplastia produce resultados clínicos y estéticos favorables en los pacientes aquejados por esta patología.


The eyes are the main focus of the face and the expression of the individual's personality is attributed to them. The anatomical configuration of the eyelids and the periorbital region constitutes one of the most important areas of human expression. There are apparently aesthetic alterations that lead to visual disorders, such as dermatochalasis and blepharochalasis, which can limit the visual field or cause eye fatigue due to the redundant pressure of the skin on the eyelashes, the laxity of the canthal tendons causes a scleral exposure syndrome and at the same time tearing. Blepharoplasty produces favorable clinical and aesthetic results in patients suffering from dermatochalasis.

ABSTRACT

Objetivo: Describir los resultados funcionales de la blefaroplastia superior. Métodos: Se realizó un estudio descriptivo longitudinal prospectivo de 99 pacientes (198 párpados) con diagnóstico de dermatochalasis de párpado superior, algunos asociados a ptosis palpebral y de la ceja, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", en el período comprendido de febrero del año 2019 a enero de 2020. Resultados: El 85,8 por ciento fueron del sexo femenino y el 43,5 por ciento eran mayores de 62 años. Predominó la dermatochalasis del párpado superior y la ptosis palpebral en el 68,4 por ciento, así como la dermatochalasis y la ptosis palpebral severa, las cuales representaron el 47,5 y el 50 por ciento respectivamente. En el grupo de 56 a 61 años sobresalió la dermatochalasis del párpado superior y la ptosis de la ceja, con el 61 por ciento. En el 98 por ciento de los casos intervenidos se obtuvo la corrección total de la dermatochalasis del párpado superior, la corrección de la ptosis en el 86 por ciento y de la ceja en el 88 por ciento. No se presentaron complicaciones en el 94 por ciento de los párpados intervenidos. La complicación más frecuente fue el sangramiento, con el 2,5 por ciento. Conclusiones: El abordaje de la ptosis palpebral y de las cejas transblefaroplastia superior logra en la mayoría de los pacientes la corrección total de estas afecciones y de la dermatochalasis. Este procedimiento constituye una herramienta importante para el rejuvenecimiento facial(AU)


Objective: Describe the functional results of upper blepharoplasty. Methods: A prospective longitudinal descriptive study was conducted of 99 patients (198 eyelids) diagnosed with upper eyelid dermatochalasis, in some cases associated to eyelid and eyebrow ptosis. The study was carried out at Ramón Pando Ferrer Cuban Institute of Ophthalmology from February 2019 to January 2020. Results: Of the total patients studied, 85.8 percent were female and 43.5 percent were aged over 62 years. Upper eyelid dermatochalasis and palpebral ptosis prevailed with 68.4 percent, whereas severe dermatochalasis and palpebral ptosis represented 47.5 percent and 50 percent, respectively. A predominance was found of upper eyelid dermatochalasis and eyebrow ptosis in the 56-61 age group (61 percent). Total correction of upper eyelid dermatochalasis was achieved in 98 percent of the cases intervened, eyelid ptosis correction in 86 percent and eyebrow ptosis correction in 88 percent. No complications appeared in 94 percent of the eyelids intervened. The most common complication was bleeding with 2.5 percent. Conclusions: In most cases, management of palpebral ptosis and upper eyebrow transblepharoplasty achieves total correction of these disorders and dermatochalasis. The procedure is an important tool for facial rejuvenation(AU)

Introducción: las malposiciones palpebrales (ptosis, dermatochalasis superior y ectropión) son de los principales motivos de consulta en el área de cirugía plástica ocular. Aún no hay información concluyente en la literatura sobre los cambios corneales topográficos que se generan con las malposiciones palpebrales y si hay cambios en la topografía corneal posterior a la corrección quirúrgica de estas. Objetivo: determinar los cambios corneales topográficos de los pacientes con malposiciones palpebrales sometidos a corrección quirúrgica. Diseño del estudio: estudio observacional prospectivo longitudinal. Método: pacientes seleccionados por conveniencia a quienes se les realizó corrección quirúrgica de afecciones palpebrales (ptosis, ectropión y dermatochalasis superior) en el Hospital Militar Central de Bogotá entre abril y septiembre de 2019. Se describieron los datos de la topografía corneal previa a la corrección y al mes y tres meses después de esta y la agudeza visual mejor corregida preoperatoria y en la última evaluación postoperatoria realizada. Resultados: se realizó corrección quirúrgica de malposiciones palpebrales a 106 ojos de 54 pacientes. El cilindro medio tuvo un cambio de 0,13 D y el desplazamiento de su eje de 1, 49º. El cambio global en la queratometría media fue de 0,01 D y el grosor corneal central disminuyó 1,5 mcs. En cuanto la agudeza visual mejor corregida hubo un aumento de 0,0415 por escala LogMAR. Conclusión: la cirugía de las malposiciones palpebrales genera cambios topográficos corneales, donde la corrección de ptosis es la que más cambios ocasiona, con persistencia de los cambios a los 3 meses postoperatorio.


Background: palpebral malpositions (ptosis, upper dermatochalasis and ectropion) are the main reasons for consultation in the area of eye plastic surgery. There is still no conclusive information in the literature on the topographic corneal changes that are generated with palpebral malpositions and if there are changes in the corneal topography after surgical correction. Objective: to determine the topographic corneal changes of patients with palpebral malpositions submitted to surgical correction. Study design: longitudinal prospective observational study. Method: patients selected for convenience who underwent surgical correction of palpebral conditions (ptosis, ectropion and upper dermatochalasis) at the Central Militar Hospital of Bogotá between April and September 2019. Corneal topography data prior to correction were described and one month and three months after this and the visual acuity better corrected preoperatively and in the last postoperative evaluation performed. Results: surgical correction of palpebral malpositions was performed in 106 eyes of 54 patients. The middle cylinder had a change of 0,13 D and the displacement of the cylinder axis of 1,49 °. The average global change in mean keratometry was 0,01 D and the central corneal thickness decreased 1,5 mcs. With the best corrected visual acuity there was an increase of 0,0415 per LogMAR scale. Conclusion: palpebral malpositions surgery generates corneal topographic changes, where the correction of ptosis is the one that causes the most changes, with persistence of the changes at 3 months postoperatively.

@#AIM: To evaluate the effects, advantages, disadvantages, and indications of infrabrow skin excision to correct upper eyelid dermatochalasis in middle-aged and elderly Chinese women.<p>METHODS: Sixty eyelids in 30 patients were subjected to this surgical procedure from January to June 2018. All patients had upper eyelid dermatochalasis, and the functions of their levator palpebrae were all normal. The primary indications of the procedure were upper lid dermatochalasis with lateral hooding. All 30 patients underwent infrabrow skin excision at Shanxi Eye Hospital. The degree of skin relaxation before and after surgery was evaluated in all patients, and surgical outcomes and postoperative complications were also assessed.<p>RESULTS: All patients were female, ranging from 40 to 64(mean: 53.20±7.10)years old. Skin relaxation of the upper eyelid was improved in all 30 patients. One month after the operation, 52 lids(86.7%)showed good results and 8 lids(13.33%)showed fair results. The objective Strasser system was applied to evaluate surgical results at 1 and 6mo. All patients had scores between 0 and 1 point, indicating good results. Preoperative unsatisfactory position and shape of the eyebrows were improved, and crow's feet and frontal lines were reduced. All patients were satisfied with the results. No complications related to wound dehiscence, lagophthalmos, or hypertrophic scars were observed.<p>CONCLUSION: Infrabrow skin excision is a simple, useful operation to reconstruct the upper eyelids in selected patients of Chinese ancestry. This procedure not only retains the advantages of classic blepharoplasty, but also alleviates many eyebrow problems at the same time, and maintains the original shape of the eyelid, minimizes scarring, and produces a smoother, more youthful appearance.

RESUMEN Objetivo: Evaluar los resultados quirúrgicos de la técnica de elevación de la ceja transblefaroplastia y las complicaciones presentadas. Métodos: Se realizó un estudio descriptivo, retrospectivo, de corte longitudinal, de 12 pacientes (24 párpados) con ptosis de la ceja asociada a dermatochalasis del párpado superior desde mayo a septiembre del año 2018, en la consulta de Cirugía Plástica Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer". Fueron excluidos los que ya tenían una cirugía anterior del tercio facial superior. Resultados: El 83,3 por ciento de los pacientes fueron del sexo femenino. En cuanto a los grupos de edades resultó más frecuente el grupo de mayores de 62 años de edad. Hubo un predominio de la raza blanca, para un 58,3 por ciento. En el 60 por ciento de los casos del sexo femenino (6 pacientes), la dermatochalasis del párpado superior, asociada a la ptosis de la ceja, fue moderada. La ptosis de la ceja resultó corregida en 18 párpados, para el 75 por ciento. Se presentó el 25 por ciento de complicaciones (3 pacientes). Conclusiones: La técnica de elevación de la ceja transblefaroplastia logra la corrección de la ceja en el mayor número de pacientes con un bajo porcentaje de complicaciones, y constituye una buena opción quirúrgica para este tipo de afección palpebral(AU)


ABSTRACT Objective: To assess the surgical outcomes of the eyebrow lift technique of transblepharoplasty and the complications presented. Methods: A descriptive, retrospective, longitudinal study was carried out with 12 patients (24 eyelids) with eyebrow ptosis associated with upper eyelid dermatochalasis, from May to September 2018, in the Ocular Plastic Surgery office of Ramón Pando Ferrer Cuban Institute of Ophthalmology. Those who already had a previous surgery of the upper facial third were excluded. Results: 83.3 percent of the patients were female. Regarding the age groups, the group of people over 62 years of age was more frequent. There was a predominance of the white race, accounting for 58.3 percent. In 60 percent of female cases (6 patients), dermatochalasis of the upper eyelid, associated with eyebrow ptosis, was moderate. Eyebrow ptosis was corrected in 18 eyelids, for 75 percent. 25 percent of complications were presented (3 patients). Conclusions: The eyebrow lift technique of transblepharoplasty achieves eyebrow correction in the greatest number of patients with a low percentage of complications, and constitutes a good surgical option for this type of eyelid condition(AU)

ABSTRACT

Las cejas y el párpado superior están íntimamente relacionados, por lo que se recomienda realizar un plan quirúrgico basado en la evaluación de las cejas y del área periorbitaria para lograr los resultados deseados de una apariencia descansada y natural. Para el levantamiento de las cejas se han realizado diferentes técnicas. Cada técnica tiene sus ventajas, desventajas e indicaciones. Se realizó una búsqueda bibliográfica en bases de datos que ofrece Infomed, como Ebsco, Hinari, Pubmed, Google académico y textos básicos de Oftalmología y Cirugía Estética. La elevación de la ceja mediante la blefaroplastia produce beneficios, tales como un abordaje menos invasivo, cicatrices posteriores menos evidentes, un plano de disección más fácil que disminuye el sangrado transquirúrgico, la disminución del tiempo de cirugía y por consiguiente menores complicaciones posoperatorias. Además, permite que los resultados sean duraderos, al igual que los obtenidos en la cirugía de elevación de las cejas mediante técnicas endoscópicas, con una marcada disminución en los costos al no utilizar el endoscopio. La presente revisión se realizó con el objetivo de ampliar los conocimientos sobre el envejecimiento del tercio superior facial, así como el empleo de la blefaroplastia ampliada como una alternativa quirúrgica de rejuvenecimiento periocular(AU)


The eyebrows and upper eyelids are closely related structures. It is therefore recommended to develop a surgical plan based on evaluation of the eyebrows and the periorbital area in order to achieve the desired result of a rested, natural appearance. Different techniques have been used for eyebrow lifting. Each technique has advantages, disadvantages and indications. A bibliographic search was conducted in the databases offered by Infomed, such as Ebsco, Hinari, Pubmed, Google Scholar, as well as ophthalmology and aesthetic surgery basic textbooks. Eyebrow lift by transblepharoplasty has benefits such as a less invasive approach, less visible scars after surgery, an easier dissection plane reducing intraoperative bleeding, reduced surgery time and therefore fewer postoperative complications. On the other hand, results are as lasting as those obtained by eyebrow lift surgery by endoscopic techniques, but costs are markedly lower due to the non-use of an endoscope. The present review was conducted with the purpose of broadening our knowledge about aging of the facial upper third, as well as about the use of extended blepharoplasty as a surgical alternative for periocular rejuvenation(AU)

Introducción: La blefaroplastia produce resultados clínicos y estéticos favorables en los pacientes aquejados por dermatocalasis. Objetivo: Evaluar los resultados de los procedimientos quirúrgicos convencional y con láser de CO2 en estos afectados. Métodos: Se realizó un estudio descriptivo, prospectivo y trasversal de 300 pacientes con dermatocalasis, atendidos en el Servicio de Oculoplastia del Centro Oftalmológico del Hospital General Docente Dr Juan Bruno Zayas Alfonso de Santiago de Cuba, desde enero de 2015 hasta diciembre de 2016. Resultados: Predominaron el grupo etario de 40 a 59 años, el sexo femenino y la afectación en los párpados superiores, sin diferencias significativas en cuanto a las técnicas quirúrgicas empleadas; se observó un mayor número de complicaciones en los pacientes operados de manera convencional, con más frecuencia del sangrado. En general los pacientes evolucionaron satisfactoriamente y los que presentaron limitaciones del campo visual superior se recuperaron sin dificultad luego de la intervención quirúrgica. Conclusiones: La blefaroplastia con láser de CO2 constituye una mejor y más calificada opción terapéutica para la corrección de la dermatocalasis


Introduction: Blepharoplasty produces favorable clinical and cosmetic results in the patients that are suffering from dermatochalasis. Objective: To evaluate the results of the conventional and CO2 laser surgical procedures in these affected patients. Methods: A descriptive, prospective and cross-sectional study of 300 patients with dermatochalasis, assisted in the Oculoplasty Service of Dr Juan Bruno Zayas Alfonso Teaching General Hospital Ophthalmologic Center was carried out in Santiago de Cuba, from January, 2015 to December, 2016. Results: There was a prevalence of the 40 to 59 age group, female sex and upper lids disorder, without significant differences concerning the surgical techniques that were used; a higher number of complications in patients operated with a conventional way was observed, with more frequency of bleeding. In general the patients had a favorable clinical course and those that presented limitations of the upper visual field recovered without difficulty after the surgical intervention. Conclusions: Blepharoplasty with CO2 laser constitutes a better and more qualified therapeutic option for the correction of dermatochalasis

Objective To explore the clinical classification of upper lid dermatochalasis in mid dle-aged and elderly women for choosing appropriate surgical methods and evaluating the efficacy of the treatment.Methods A lot of 98 cases of middle-aged and elderly women above 40,who underwent surgical treatment between January 2005 and September 2015,were retrospectively analyzed.The dermatochalasis was classified according to the relaxation of upper lid soft tissue,eyebrow ptosis and the effect of eye function.Therefore,four surgical treatments were designed for patients-upper eyelid incision,infraeyebrow incision,eyebrow lifting and upper eyelid incision plus eyebrow lifting.Results Of the 133 treated cases,the post-operative cosmetic result was assessed as very satisfactory in 107 (80.4%) cases;as satisfactory in 19 (14.3%) cases,and as dissatisfactory in 7 (5.3%) cases.Conclusions The key to satisfactory treatment of upper lid blepharoplasty for middle aged and elderly women lies in designing personalized treatments and choosing appropriate surgical methods according to the classification of upper lid relaxation.

ABSTRACT

BACKGROUND: To date, many methods have been proposed to resolve the sunken eyelid. We treated our clinical cases of sunken upper eyelid based on whether there is a concurrent presence of the dermatochalasis or blepharoptosis and then performed the autologous fat grafting using either closed or open technique. METHODS: According to the classification of the patients, we used the following injection techniques: First, we solely performed the autologous fat grafting using the closed technique in the sunken upper eyelid only group, for which we did the injection using a micro-injector cannula on the suborbicularis plane. Second, we performed such procedures as skin excision, orbicularis oculi muscle strip excision and levator aponeurosis advancement to manage each symptom in the sunken upper eyelid with dermatochalasis or blepharoptosis group. Simultaneously, we also made a small window in the lateral portion of the orbital septum using the open technique with metzenbaum scissors. Thus, we performed the intraseptal injection of the autologous fat. RESULTS: During a period ranging from January of 2012 to April of 2014, we treated a total of 53 patients. The patients were followed up during a mean period of four months. Thus, we obtained satisfactory treatment outcomes without notable complications. CONCLUSIONS: In conclusion, our results indicate not only that surgeons should consider the fat grafting as one of eyelid-surgery procedures but also that they should perform it concurrently with blepharoplasty or blepharoptosis correction for the purposes of obtaining good treatment outcomes.

ABSTRACT

Objetivo: presentar los resultados de la cirugía de blefaroplastia, con la técnica convencional y con el uso del láser de CO2. Método: se realizó un estudio descriptivo longitudinal y prospectivo en 50 pacientes que fueron atendidos en el Servicio de Oculoplastia del Instituto Cubano de Oftalmología Ramón Pando Ferrer, con el diagnóstico de Dermatochalasis y/o hernia del tejido adiposo orbitario, de enero a junio del 2006. Se conformaron dos grupos de 25 pacientes seleccionados de forma aleatoria, uno para ser intervenido con láser y otro por la técnica convencional. Resultados: el mayor número de pacientes tratados quirúrgicamente por Dermatochalasis correspondió al grupo de edades entre 50 a 60 años, predominó la cirugía convencional. El 64,0 por ciento representaba al sexo femenino, se le aplicó dentro de este grupo la técnica convencional a un 68,0 por ciento. El 68,0 por ciento correspondió a pacientes de piel blanca, con un uso de 76 por ciento de láser de CO2. El 58,0 por ciento de estos pacientes se le realizó la técnica quirúrgica en un tiempo comprendido entre 30 y 45 minutos. Las complicaciones fueron hiperpigmentación al aplicar el láser de CO2 y la hipocorrección al utilizar la técnica convencional. Conclusiones: ambas técnicas quirúrgicas muestran resultados satisfactorios para el paciente, por lo que se convierten en una solución al problema estético y funcional del mismo, pero con el uso del láser de CO2 se redujo las complicaciones y el tiempo quirúrgico, siendo así la alternativa preferida por médicos y pacientes


Objective: to present the results of blepharoplasty with the conventional technique and with CO2 laser. Method: prospective, longitudinal and descriptive study of 50 patients with diagnosis of dermatochalasis and/or orbital fatty tissue hernia, who were assisted at the Oculoplasty service of Ramon Pando Ferrer Cuban Institute of Ophthalmology from January to June 2006. Two groups of 25 randomly selected patients were created; one to be operated on with laser and the other with the conventional procedure.Results: the highest number of patients surgically treated from dermatochalasis lied in 50-60 y age group and the conventional surgery was predominant. Females accounted for 64 percentv and in this group, the conventional technique was performed in 68 percent of patients. Sixty eight percent of patients were Caucasians, with use of CO2 laser amounted to 76 percent. The surgical time for 58 percent of these patients who underwent the surgical technique ranged 30 to 45 minutes. The complications were hyperpigmentation when using laser and hypocorrection when using the conventional technique. Conclusions: both surgical techniques show satisfactory results for the patients, so they become solutions to their esthetic and functional problems; however, CO2 laser reduced complications and the surgical times, so it is the alternative of choice for physicians and patients alike

ABSTRACT

PURPOSE: To describe a modified surgical technique for blepharoptosis repair through a small orbital septum incision and minimal dissection, along with the results obtained in patients with coexisting dermatochalasis. METHODS: A retrospective chart review included 33 patients (52 eyelids) with blepharoptosis coexisting with dermatochalasis, surgically corrected through a small orbital septum incision and minimal dissection after redundant upper lid skin excision, by placing a single fixation suture between the levator aponeurosis and the tarsal plate. Outcome measures included the pre- and postoperative marginal reflex distances (MRD1), eyelid contour, post-operative complications, and need for reoperation. RESULTS: The pre- and postoperative MRD1 averaged 1.1 +/- 0.8 mm and 2.8 +/- 1.1 mm, respectively. Of the 33 patients, 9 patients (9 eyelids) underwent surgery on one eyelid for unilateral blepharoptosis and dermatochalasis (Group I), 5 patients (5 eyelids) underwent a simple skin excision blepharoplasty of the contralateral eyelid (Group II), and 19 patients (38 eyelids) underwent bilateral blepharoptosis and dermatochalasis repair (Group III). Of the 14 eyelids that underwent unilateral ptosis repair (Groups I and II), 12 eyelids (85.7%) showed less than a 1-mm difference from the contralateral eyelid. Of the 38 eyelids that underwent bilateral ptosis repair (Group III), 27 eyelids (71.1%), 5 eyelids (13.1%), and 6 eyelids (15.8%) had excellent, good, and poor outcomes, respectively. Overall, 44 eyelids (84.6%) out of a total of 52 eyelids had successful outcomes; the remaining 8 eyelids demonstrated unsatisfactory eyelid contour was corrected by an additional surgery. CONCLUSIONS: Blepharoptosis repair through a small orbital septum incision and minimal dissection can be considered an efficient technique in patients with ptosis and dermatochalasis.

ABSTRACT

PURPOSE: To evaluate histopathologic properties of eyelid skin and conjunctiva and the association between clinical manifestation and histopathologic findings. METHODS: A prospective study was conducted on the histologic findings of the upper eyelid skin and conjunctiva performed from November 2009 to February 2010 in 27 patients for upper eyelid blepharoplasty procedures. Histopathologic studies were performed for specimens collected from the upper eyelid skin and inferotemporal bulbar conjunctiva. Preoperative photographs of the upper eyelid and conjunctiva were taken to grade clinical severity of dermatochalasis and conjunctivochalasis. RESULTS: Decrease of collagen density and elastic degeneration in the eyelid skin and conjunctiva were significantly associated with aging (p < 0.001, p = 0.001, p = 0.001, p < 0.001). Clinical severity of dermatochalasis was correlated with a decrease of collagen density and elastic degeneration in the eyelid skin, and clinical severity of conjunctivochalasis was associated with a decrease of collagen density, elastic degeneration and lymphangiectasia in conjunctiva (p < 0.001). CONCLUSIONS: Change of collagen fiber and elastic fiber in the eyelid skin and conjunctiva was observed with aging. Relaxation of the eyelid skin and conjunctiva was correlated with histologic findings.

In the group with brow ptosis, only one patient felt a significant change in brow position after blepharoplasty. In both groups, the distance between the eyebrow and the lid margin decreased by similar amounts. There was no change in the upper eyelid margin position after blepharoplasty. CONCLUSIONS: In patients with dermatochalasis, upper eyelid blepharoplasty caused a lowering of the eyebrow without a change in the eyelid margin. It is important to evaluate the brow ptosis preoperatively and excise the correct amount of excess skin to avoid aggravation of brow ptosis.

ABSTRACT

A survey on the incidence of blepharo-dermatochalasis on Filipinos was made in a 5 year period on 2500 persons past middle age (40-60); 2200 were army personnel and 300 were civilians. The finding was an incidence of 68 percent, of which 66 percent were of primary cause and 2 percent of secondary causes. Out of 204 civilian cases, only 47 were operated on (23.04%). No army personnel were operated on. A classification is submitted with comparative sketches for those eyelids with fold and those without. Technique on how to estimate the excess of skin to be excised in both classifications and their surgical managements are presented. Blepharo-dermatochalasis which is found common among persons past middle age (40-60) causes cosmetic appearance to suffer in all degrees. However, the surgical management as presented, which is simple and without danger to the eyes, gives pleasing cosmetic results.
